sorry the name of the form is the advanced anxious thought form  this is what i exactly was talking about from the begining .

The anxiety form is not tracked by the program right?as we do not attach it to the program

Welcome to the group!
 
All the forms are for your benefit and tracking. Some members share their tracking forms with their doctors but it is not required that you complete the forms with a doctor. This is a personal preference.

Hello... this is my first day in this program... and im very excited !!! i hope to feel better as soon as possible ... i started the 1st session; and i was wondering about the anxiety form; should i make it in a doc. daily and then send it to that email?!! or is it like a diary for me to keep?!  i didnt get it .. help plz!!!
